Title: QIDP Administrator

Summary/Objective:

 

Responsible for the care of each resident in assigned homes through the management of individual care, home finances and staff. Implements and facilitates each resident’s Service Plans and provides the necessary ongoing instruction and support services to maximize individuals’ abilities to live successfully as integrated members of the community. Responsible for the Instruction, supervision and assistance of individuals through staff involvement in skills based on their ISP’s, which allows for greater strengths and independence in their lives. These services should be person centered and focused on individualization, integration, independence and productivity.
Essential Functions:

  •  Ensure that each resident’s interests, needs, care and desires are addressed in keeping with Salem’s philosophy of individualization and people first so that each person can reach their potential for independence.
  • Function as a member of the interdisciplinary team in the formulation and the execution of Individualized Service Plans (ISP’s).
  •  Serve as Salem’s liaison with parents, schools, regional centers, day programs, case managers, and other individual groups and agencies as necessary.
  • Monitor resident charts, medications, physical environment and P&I to assure that Salem meets the highest standards of quality care.
  • Responsible for safeguarding effective care results by maintaining Salem policies, procedures and forms, ensuring that staff have been trained in their use and holding staff accountable for their performance.
  • Responsible for the training and orientation of staff in medication administration, restricted health care plans, CPI and vital signs. Training to include the concepts and staff acceptance of individualization, integration, independence and productivity. Work with HR to ensure that all training is completed and tracked per Salem policies and timelines.
  • Responsible for the creation and maintenance of home schedules to assure effective use of staff to meet resident needs. Ensure that the daily, weekly and/or monthly staff schedule at each of the Salem facilities are filled with appropriate staff based on resident needs, state and local regulations, census and availability. Working with the Direct Care Home Supervisors, will be responsible for providing staff coverage for call-offs, scheduling conflicts, emergencies, holidays, vacations, and leave of absences.
  • Writes or reviews reports monthly, quarterly, semi-annually for assigned individuals as required while ensuring timely completion.
  •  Perform all necessary supervisory functions to effectively and efficiently manage the personnel assigned. Maintains a qualified staff. Communicates areas of accountability and performance expected of personnel assigned. Determines standards of performance as a basis to review progress of personnel assigned. Recommends salary adjustments. transfers, promotions and dismissals. Fosters a cooperative and harmonious working climate conducive to maximizing employee morale and productivity.
Required Education and Experience:

 

  • Bachelor’s degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling or psychology
  • At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
  •  At least three years of management experience.
Preferred Education and Experience:

 

  • Master Degree in Psychology, Sociology, Special Education and Behavioral Science.
  • Applied Behavior Analysis (ABA) certificate.

Title: Direct Care Home Supervisor 2
Location: Residential Care Facilities
Summary Lead others in providing superior residential services to adults with developmental disabilities. The DCHS2 is a Community Care Licensed (CCL) Level 4A-4I or Intermediate Care Facility (ICF) home supervisor and experienced care provider. The DCHS2 will help guide others and provide residents with living skills development, health maintenance, personal care, and behavioral development.
Qualifications
  • Excellent communication skills; respectful and positive attitude toward residents, co-workers, contractors, and home visitors.
  • Administrator Certificate
  • Have completed Salem’s supervisory training course or show prior supervisory experience compatible with the position.
  • Able to lead, motivate and supervise staff and make good decisions to provide the best care possible to residents.
  • Intermediate computer skills including MS Office Suite – Word, Excel & Outlook
  • Complete required staff training and a minimum of 12 hours continuing education annually.
  • Complete orientation and on-the-job training within the first month of employment.
  • Work weekend, holiday, and overnight shifts as required.
  • Must have a cell phone and participate in Salem Cell Phone Policy – current reimbursement is $15.00 per month.
  • Able to meet Title 22 requirements, including, but not limited to:

~  Minimum age 18

Job Information Staff duties include dressing, bathing, hygiene, cooking, housekeeping, community activities, medical appointments, administering medication and treatments, active treatment following therapist orders, completing daily documentation and participating in (Interdisciplinary) ID team meetings. Minimum staff requirements include High school education or equivalent, orientation and 36 in-service hours per year.  Additional training may be required.
Qualifications
  • Be able to legally work in the United States;
  • Be 18 years of age; must meet the qualifications of a driver as established by our insurance carrier, Department of Motor Vehicles and Salem policy;
  • Have a high school diploma or GED, preferably
  • Be able to pass a background check and receive a fingerprint clearance from the Department of Community Care Licensing and Department of Health Services
  • Be able to provide verifiable non-family references including 1 previous employer.
  • Be able to pass a Physical/TB/PAT/Drug Screen
